If you student is eligible to sign up for athletics and chose to register them for a sport, they will need to complete and return the Concussion Information Sheet below BEFORE their FIRST practice.
Si su estudiante es elegible para inscribirse en actividades deportivas y usted decide registrarlo en algún deporte, deberá completar y entregar Concussion Information Sheet que se encuentra a continuación ANTES de su PRIMER entrenamiento.
The form below is needed to administer ANY medical related assistrance to students (even Cough drops). If you want your student(s) to receive treatment from the office during the school year, fill out this form and return it in your Family Envelope.
El formulario que aparece a continuación es necesario para administrar CUALQUIER tipo de asistencia médica a los estudiantes (incluso pastillas para la tos).
